There are really just two things you need to do to begin your course of therapy at maXum Physical Therapy.

First, you'll need to have your doctor fill out a maXum Physical Therapy prescription form. You can download and print the necessary prescription here

The second step is to download, print and fill-out the various information and insurance forms you normally need to fill out on your first office visit. By filling the forms out beforehand, your first office visit will be more relaxed and your wait will be shorter. Download all the necessary forms here.

Please note, downloading the forms is not mandatory and is only suggested for your convenience. All necessary forms can be filled out at the maXum Physical Therapy office at the time of your first visit.
